Service Overview

Forms People Can Actually
Finish on the First Try

"A form is a conversation. Most bad forms fail because nobody thought about the order of the questions."

SBTEXMEDIA designs professional fillable PDF forms for organisations that need information collected accurately, on any device, without an administrator retyping it afterwards. Led by Bayazid Kajol, founder of SBTEXMEDIA, this service covers everything from a two-page client intake form to a multi-section application pack with automatic calculations, conditional sections and legally-usable digital signatures.

Building a genuinely good fillable form is a structural discipline rather than a design one. Every field needs a sensible internal name so the exported data is readable. The tab order has to follow the visual layout, because administrators fill hundreds of these without ever touching a mouse. Required fields need validation that guides rather than scolds. Totals should calculate themselves. Sections that do not apply should disappear. And all of it must behave identically in desktop Adobe Acrobat, in a phone's PDF viewer and inside a browser preview — three environments that render form fields very differently.

I build in Acrobat Pro at field level, using JavaScript for calculations and validation where it genuinely helps. Where the starting point is a scan or a document with no source file, the PDF editing work comes first so the form is built on clean foundations. And where a process would genuinely be better online, I will tell you so — sometimes Google Forms or Jotform is the right answer, and recommending the wrong tool to win a job helps nobody.

The Real Cost

Why Fillable PDF Forms
Matter

A badly built form charges a tax to every single person who touches it — and that cost never appears on an invoice.

Almost every business runs at least one process on a form that does not work properly. The registration pack that applicants print, complete by hand, photograph on a phone and email back. The order sheet where the totals are added manually and occasionally wrong. The consent form that gets returned three times because a required field was easy to miss. None of these are dramatic failures, which is precisely why they survive for years.

The cost is real and measurable, though. Every hand-completed form that arrives as an image has to be retyped by someone, and every retyping introduces a chance of error. A team processing forty submissions a week at four minutes of manual data entry each is losing well over two hours weekly to a problem that a properly-built form eliminates completely. Multiply that across a year and the form has quietly cost more than any professional would have charged to fix it.

Then there is the abandonment cost, which is harder to see because the people it affects never contact you. A form that will not open on a phone, that resets when the respondent taps the wrong thing, or that demands information in a confusing order simply does not get completed. You do not receive a complaint — you receive nothing at all, and you have no way of knowing how many enquiries you lost to it.

There is also a compliance dimension that catches organisations out. In regulated sectors, a form is a record. If it cannot be completed by someone using a screen reader or navigating by keyboard alone, that is an accessibility failure. If a signature field is decorative rather than a real signature field, the document may not hold up when it is questioned. If exported data arrives with field names like Text1 and Untitled5, nobody can audit it later.

A properly engineered fillable PDF removes all of it. The respondent completes it on whatever device they have, guided by validation and logical order. Totals calculate themselves. Signatures are genuine. The data arrives structured, named and ready to use. And because the file is built to be maintained, next year's version is a small edit rather than another rebuild.

What You Get

Benefits of Professional Form Design

Six outcomes that separate an engineered form from a document with boxes drawn on it.

01
⌨️
Logical Tab Order
Tab moves through the form the way the eye reads it — down a column, across a row, never jumping unpredictably. Administrators completing dozens of these daily will notice within thirty seconds.
02
🧮
Automatic Calculations
Subtotals, totals, tax, discounts, percentages and multi-field arithmetic calculate themselves as the form is completed. No mental maths, no calculator, no arithmetic errors arriving in your inbox.
03
Validation That Guides
Email, phone, date, currency and required-field checks catch mistakes at the moment they happen, with messages that explain the fix rather than simply refusing to continue.
04
📊
Clean, Named Data Export
Fields are named meaningfully, so exported CSV, FDF or XML arrives with readable column headings you can sort, filter and analyse immediately instead of decoding.
05
✍️
Real Signature Fields
Genuine signature fields supporting digital certificates and drawn signatures, with optional locking so the document cannot be altered after it has been signed.
06
📱
Works on Every Device
Tested in desktop Acrobat, on iOS and Android viewers, and in browser preview — the three places forms most often break. What you approve is what your respondent actually meets.
Is This For You?

Who Needs Fillable PDF Forms

If any of these describe your week, this is the service you are looking for.

01
⌨️
Your Team Retypes Submissions
Forms come back handwritten, scanned or photographed, and somebody has to key the contents into a spreadsheet or system. That entire step can disappear.
02
🖨️
You Still Rely on Print-and-Sign
Clients must own a printer, a pen and a scanner to do business with you. Every one of those requirements loses you a percentage of respondents.
03
🧾
Your Forms Involve Numbers
Order forms, quotes, invoices, timesheets and expense claims where totals are currently added by hand — and occasionally added wrong.
04
🔁
You Chase Incomplete Submissions
Forms arrive missing required information, so every submission triggers a follow-up email. Validation stops that at source rather than after the fact.
05
🏛️
You Work Under Compliance Rules
Healthcare, legal, financial or government contexts where forms are records and must be accessible, auditable and properly signed.
06
📴
You Need Forms That Work Offline
Site inspections, field surveys and clinical settings where an online form is not an option, but you still need structured, exportable data.
Sector Experience

Industries Served

Twelve-plus sectors, each with its own rules about consent, signatures, retention and who may see the data.

A medical intake form and a construction inspection sheet can look like the same object while answering to completely different requirements around consent wording, signature validity, record retention and data handling. Knowing those differences in advance is what stops a form being rebuilt after a compliance review. Across the sectors below I have enough working literacy to ask the right questions at the briefing stage rather than discovering the constraint halfway through the build.

How It Works

Our Process

The same six steps on a one-page consent form and a forty-page application pack.

Form projects fail in a specific way: everyone agrees on the fields, nobody discusses what happens to the data afterwards, and the finished form turns out to export something the client's system cannot read. My process front-loads those questions deliberately, so the awkward discoveries happen during the brief rather than after delivery.

1
Discovery & Data Mapping
Before quoting I ask who completes the form, on what device, what happens to the data afterwards, and which fields are legally required. This is where the real requirements surface — the system that needs a specific CSV column order, the regulator who mandates particular consent wording.
2
Written Scope & Fixed Quote
You receive a written summary listing every field, the logic and calculations involved, the export format, revision rounds and a delivery date, at a fixed price. Nothing starts until you approve it.
3
Document Preparation
The underlying document is cleaned or rebuilt first — scans are OCR'd, spacing is corrected, and field areas are sized to fit realistic answers rather than optimistic ones. A form built on a messy base is always a fragile form.
4
Field Engineering
Fields are created with meaningful internal names, then wired up: validation rules, formatting masks, dropdown values, calculations, conditional show-and-hide behaviour, signature fields, and a tab order that follows the visual reading order exactly.
5
Multi-Environment Testing
The form is completed end to end in desktop Acrobat, on iOS and Android, and in browser preview. Every calculation is verified against manual arithmetic, every conditional rule triggered in both directions, and a real test submission exported to confirm the data lands correctly.
6
Delivery, Handover & Aftercare
You receive the finished form, a flattened print version where useful, the editable source, and a short note on updating fields yourself. Revisions are handled promptly and I answer questions long after the invoice closes.
Why Choose Us

Why Choose SBTEXMEDIA

Six reasons drawn from what returning clients say when I ask them why they came back.

01
👤
You Work Directly With the Founder
Bayazid Kajol, founder of SBTEXMEDIA, builds your form personally. No account manager, no queue, no handoff to a junior — the person who engineers your fields is the person who answers your email about them.
02
🔬
Field-Level Technical Depth
JavaScript calculations, validation scripting, conditional logic, signature configuration and export mapping. This is the work most designers decline, and it is precisely what I specialise in.
03
🧪
Tested Where Forms Actually Break
Every form is completed and submitted on desktop, mobile and browser before delivery. Most form failures are environment failures, and they are only found by testing in all three.
04
🧩
Honest Tool Recommendations
If your process would genuinely work better as a Google Form, a Microsoft Form or a Jotform, I will say so — even though it means a different quote. Recommending the wrong tool to win a job serves nobody.
05
🔐
Confidentiality as Standard
Medical intake forms, legal agreements, financial applications and HR records are handled routinely. Files stay private, are never used as portfolio pieces without written permission, and NDAs are signed gladly.

Good Questions

Frequently Asked Questions

The questions clients ask before commissioning a form. For broader questions, see the main FAQ page.

Yes — including scanned documents with no source file. Depending on the quality of the original I either build the form layer directly onto the existing PDF or rebuild the document first so the form has clean foundations. Either way you receive proper named fields, logical tab order, validation, dropdowns, checkboxes, calculations and signature fields where required.
Yes, and this is tested rather than assumed. Mobile PDF viewers handle form fields differently from desktop Acrobat — some ignore certain JavaScript, some render field borders differently, some struggle with very small tap targets. I complete every form end to end on iOS and Android before delivery, and size fields so they are comfortably tappable on a phone.
Yes. Line totals, subtotals, tax, discounts, percentages, date differences and conditional arithmetic can all be built in using Acrobat's calculation features and JavaScript where needed. Every calculation is verified against manual arithmetic during testing, including awkward cases like empty fields, zero values and negative numbers, which is where most calculated forms quietly break.
Yes. I add genuine signature fields rather than image placeholders, supporting both digital certificate signatures and drawn signatures depending on what your situation requires. The document can be configured to lock automatically once signed, so nothing can be altered afterwards. If your sector has specific e-signature requirements, tell me at the briefing stage and I will build to them.
Several ways, depending on your setup. Completed forms can be emailed back and the data exported in bulk to CSV for spreadsheet analysis, or exported as FDF or XML for import into another system. Forms can also submit directly to an email address or a web endpoint. Because fields are named meaningfully, your export arrives with readable column headings rather than Text1 and Untitled5.
Conditional logic shows and hides sections based on earlier answers — so someone who ticks "no" to a question never sees the eight follow-up fields that only apply to "yes". It shortens the form dramatically for most respondents and improves completion rates. You need it whenever your form currently says "if you answered no, skip to section 4", which is a reliable sign the form is doing work the file should be doing.
It depends on the process, and I will give you a straight answer rather than the one that suits my invoice. Fillable PDFs win when the document must be printed, archived, signed, kept as a formal record, or completed offline. Online forms win when you want submissions flowing automatically into a spreadsheet or CRM. See Google Forms, Microsoft Forms and Jotform — and plenty of clients end up using both.
Yes, and it is included rather than charged as an extra. Every field gets a meaningful label and tooltip, the document is tagged with a logical reading order, contrast is checked, and the whole form is tested using keyboard navigation alone from first field to submit. If a form cannot be completed without a mouse, I do not consider it finished.
Simple forms of one to three pages are typically delivered within 24 to 48 hours. Larger packs with calculations, conditional logic and multiple signature stages usually run three to seven days. Long application systems built across several linked documents can take up to two weeks. Every deadline is agreed in writing, in your time zone, before work begins.
Yes, if you have Adobe Acrobat Pro. I deliver the editable source with clean field naming and a short handover note explaining how the fields, calculations and logic are structured, so straightforward changes like updating a dropdown list or amending wording are things your team can do without me. For anything structural I am available, but I would rather you were not dependent on me for small edits.
